Indeed, one of the limitations of the study was that human cells are not exposed to e-liquids per se, but rather to the aerosols where the concentrations are lower [86]. In this line, the maximum concentration tested (1000 µM) would correspond to approximately 80 to 150 ppm, which is far higher than the levels found in aerosols of some of these compounds [84]. Moreover, on a day-to-day basis, lungs of e-cigarette users are not constantly exposed to these chemicals for 24 h at these concentrations. Similar limitations were found when five of seven flavourings were found to cause cytotoxicity in human bronchial epithelial cells [87]. It is well known that nicotine is extremely addictive and has a multitude of harmful effects. Nicotine has significant biologic activity and adversely affects several physiological systems including the cardiovascular, respiratory, immunological and reproductive systems, and can also compromise lung and kidney function [41].
Several states have imposed restrictions on the sale of e-cigarettes. There are also restrictions on liquids with flavors that may be more attractive to younger people. The researchers also noted a lack of evidence that vaping is an effective means of quitting smoking. The marketing of e-cigarettes and their range of flavors can give the impression that vaping is not harmful.

The e-liquid is quickly heated and converted to an aerosol that can be inhaled into the lungs. As smoking has declined dramatically in the U.S., vaping has gained tremendous popularity very quickly. A 2018 study reported 20% of high schoolers vaped at least once in the past 30 days. Since e-cigarettes often contain the same chemicals as traditional cigarettes, smoke produced by vaping may be toxic to people nearby. Manufacturers claim that e-cigarettes bypass many of the health risks of tobacco smoking, providing a healthful alternative.
Youth ENDS use raises concerns about nicotine addiction, negative effects of nicotine on adolescent brain development, and other potential health harms, including increased risk of initiating cigarette smoking. Electronic nicotine delivery systems (ENDS), also called electronic cigarettes, e-cigarettes, vaping devices, or vape pens, are battery-powered devices used to smoke or "vape" a flavored or unflavored solution which usually contains nicotine. The American Academy of Family Physicians (AAFP) recognizes the increased use of ENDS, especially among youth and young adults, as well as its use by those attempting to quit smoking tobacco. Although e-cigarettes do not contain tobacco, for regulatory purposes, they are considered "tobacco products" by the US Food and Drug Administration (FDA) and the Centers for Disease Control and Prevention (CDC). E-cigarettes can go by different names, including vapes, vape pens or sticks, e-hookahs, hookah sticks, mods, and personal vaporizers.
However, it has been reported that the heating process can lead to the generation of new decomposition compounds that may be hazardous [4, 5]. The levels of nicotine, which is the key addictive component of tobacco, can also vary between the commercially available e-liquids, and even nicotine-free options are available. For this particular reason, e-cigarettes are often viewed as a smoking cessation tool, given that those with nicotine can prevent smoking craving, yet this idea has not been fully demonstrated [2, 6, 7].
Parental smoking, positive attitude towards e-cigarette use in one’s age group, current smoking, and snus use increased S-EC, whereas liking school lowered it. At school level, both among girls and boys, a higher proportion of students liking school and being never-users lowered S-EC, whereas a higher proportion of students with positive attitudes towards e-cigarette use increased it. Among girls, a higher proportion of girls in the school, students with a high parental education and planning for GUSS lowered S-EC, but a higher proportion of students with parental smoking increased it.
Federal, state, and/or local governments need to set clear standards on environmentally responsible e-cigarette waste disposal and hold the industry accountable for adhering to them. E-cigarette manufacturers introduced a new tobacco product that soared in popularity with little thought on how to responsibly dispose of the resulting tons of e-cigarette waste. The group then performed a statistical analysis to determine group differences in vascular function before and after vaping.
With millions of teens and young adults using e-cigarettes, a new generation could become addicted to nicotine and face other health risks. There have also been cases of e-cigarettes exploding or catching fire. Some chronic users have developed obstructions in their lung pathways that cause them to suffer 'asthma-like' symptoms. Similar to Wyoming, the state's vast rural areas might contribute to difficulty enforcing regulations surrounding youth vaping. West Virginia came in second place, with 28 percent of teens vaping last year. The large rural area may make it more difficult to enforce the state's vaping laws. Rural areas also tend to have less access to clinics and resources about the health harms surrounding vaping.
The liquid solution can contain nicotine, flavorings, harmful chemicals, or other substances. Since the early 2000s TTCs have developed interests in e-cigarettes (also known as electronic delivery systems, or ENDS), heated tobacco products (HTPs), snus and nicotine pouches. Companies have referred to these types of product as ‘next generation products’ (NGPs) although terminology changes over time. Finally, the report discusses steps that e-cigarette companies took in 2021 to deter or prevent underage consumers from visiting their websites, signing up for mailing lists and loyalty programs, or buying e-cigarette products online. These steps include the use of online self-certification to verify users were at least 21 years old and following state laws requiring an adult signature upon delivery of e-cigarette products. The 2021 report also provides details on some characteristics of e-cigarette products, including flavors and nicotine concentration, as well as the bundling of the components in cartridge systems.
Before this study — which Truth Initiative funded — there hasn’t been any data available on how to help teens quit vaping, according to the researchers. Although advertising may make it look convenient and appealing, it’s important to know that these products deliver varying amounts of the addictive chemical nicotine, which can negatively impact your learning, attention span and proneness to addiction. Many teens are taking things a step further, adding cannabis, CBD oils and other dangerous additives to vaping devices. When patients show up to the emergency department in respiratory distress from vaping, it can be challenging for physicians to treat them due to the difficulty in correctly identifying what they inhaled, especially when they are intubated or unconscious. Funny how just about 10 years ago people were still allowed to smoke actual cigarettes in some restaurants and I didn’t see anyone walking out on their meal due to the smell coming from the smoking section. The researchers said that the findings suggest that if tobacco cigarette smokers who use e-cigarettes for a healthier alternative to tobacco won’t benefit by the switch. Cannabis and CBD e-liquids usually contain other chemicals, such as base liquids or flavoring agents. They may cause side effects similar to those of nicotine-free e-cigarettes. Young people who vape nicotine are more likely to start smoking cigarettes in the future. The 2018 NAP report concluded there’s some evidence that nicotine and nicotine-free e-cigarettes can damage oral cells and tissues in people who don’t smoke cigarettes.
Currently, e-cigarettes are not mentioned in the Illinois Smoke-free Illinois Act, which prohibits smoking in virtually all public places and workplaces, but some local ordinances do include e-cigarettes. E-cigarette aerosol is not harmless; it can contain harmful and potentially harmful substances, including nicotine and cancer-causing chemicals. While e-cigarettes typically have fewer chemicals than regular cigarettes, researchers found e-cigarettes present their own unique health risks and can increase the odds of chronic cough, phlegm, bronchitis, and asthma. In addition, while the FDA denied marketing applications for all Juul products in June 2022, it subsequently put that decision on hold in the face of a lawsuit by Juul. When e-cigarette products first became popular, we didn’t have much information about health risks.
Due to what has been called an ‘epidemic’ of youth use of these products, FDA has prioritized prevention efforts. The agency has taken a multitude of actions to keep ENDS out of the hands of youth, from policy making to enforcement to education. While e-cigarettes can generally be a lower-risk alternative for adults who smoke cigarettes, the use of e-cigarettes is not risk-free. These products deliver harmful chemicals and contain nicotine, which is highly addictive. Moreover, given the harmful chemicals found in e-cigarettes, further high-quality research on both short- and long-term health outcomes is needed. The longer ENDS and other e-cigarettes are on the market, the more information we know about their impacts on health.

Many are already working on efforts to combat this crisis, and we now ask for redoubled efforts and increased coordination. Below are recommended actions that different groups can take to address this issue. The associations between the student- and school-level independent variables and S-SM, S-EC, and S-SN were tested with two-level logistic regression models, with students on level one and schools on level two. Analyses were stratified by gender due to significant interactions between gender and educational aspirations for all three outcomes in the unadjusted analyses.
